If you have any question regarding health insurance, email osu-benefits@okstate.edu 

If you are a GRA/GTA and meet the eligibility requirements, you are enrolled automatically into the Student Health Plan with United Health Care.  

 

Once enrolled, you are sent an email from United Health Care in order to obtain your insurance card. The university typically receives reports from the Graduate College mid/late August for eligibility, coverage will be retroactive, and premiums are deducted from your paycheck.  

 

If during that time you receive medical care, be sure to keep your receipts so that you can file for reimbursement when you receive your insurance card. Click below for more information.

If you want to DECLINE health insurance, you will need to fill out a student insurance declination form and submit it to OSU Benefits (osu-benefits@okstate.edu).

Insurance declinations expire after the summer semester. If you decline in the fall, you are declined for the full academic year (unless revoked) and then will just need to re-submit your updated form the following fall. 

 

Here are some important due dates: 

Fall (23-24 Academic Year) Deadline: September 1 

2023 Spring/Summer 2024 Deadline: February 1 

2024 Summer (New Hires Only) Deadline: July 1, 2024
